The revelation that he has commenced a five-week course of radiation therapy alongside ongoing hormone treatment for his metastatic prostate cancer—characterized by a grave Gleason score of 9 (Grade Group 5) that has spread to his bones—is a necessary update to his condition. This is not just a health update as it is a critical intersection of celebrity illness, modern oncology, and the necessary conversation about healthcare access for older men.

The clinical details disclosed by Biden’s office are stark. A Gleason score of 9 places the former President’s illness in the high-grade category, meaning the cancer cells are highly abnormal and can spread rapidly.

Furthermore, the confirmation of metastasis to the bone immediately classifies the disease as advanced, or Stage IV. This is where the clinical assessment transitions from encouraging to realistic: metastatic prostate cancer is generally considered incurable.
